A community health problem is a specific issue or concern that negatively impacts the health and well-being of a specific population or geographic area. Examples of community health problems include high rates of obesity, smoking, or substance abuse, or a lack of access to healthcare services.
An improvement plan is a strategy for addressing a specific community health problem. It typically includes a clear statement of the problem, goals and objectives for addressing the problem, and specific actions or interventions that will be taken to achieve those goals. Evaluation and monitoring measures are also typically included to measure the effectiveness of the plan.
