Instructions

The final part of the Nursing Process Project requires you to complete the evaluation section on the Nursing Process Project template. Follow these steps to complete this part of the project.

Step 1: Complete the evaluation section of the Nursing Process Project template. If a category is non-applicable, simply write NA in that section of the template.

Step 2: Implement instructor comments and edits to previous sections of the Nursing Process Project template.

Step 3: Submit the entire Nursing Process Project template for final instructor review.

 

The Nursing Process Project will require you to assess a fictitious client using the nursing process. This project will be completed throughout Modules 3–8. Modules 3–7 will require you to complete an assigned part of the Nursing Process Project template. In Module 8, you will create a presentation using all of the information you compiled through the nursing process in Modules 3–7.

SOLUTION

The evaluation phase of the nursing process involves several steps:

  1. Review the goals and objectives that were established in the planning phase. These goals should be specific, measurable, achievable, relevant, and time-bound (SMART).
  2. Collect data to determine whether the goals and objectives have been met. This may include subjective and objective data, such as client feedback, physical assessments, laboratory tests, and vital signs.
  3. Compare the data collected to the expected outcomes established in the planning phase. Did the interventions have the desired effect? Did the client achieve the goals and objectives?
  4. Determine whether any adjustments to the plan of care are needed. If the goals were not met, identify the reasons why and adjust the plan of care accordingly. This may include changing interventions, modifying goals, or involving other healthcare professionals.

Example:

Let’s say that your client was a 50-year-old male who was admitted to the hospital with a diagnosis of heart failure. During the planning phase, you established the following goals:

  1. Improve the client’s heart function to reduce symptoms of heart failure.
  2. Reduce the client’s fluid retention and improve breathing.
  3. Educate the client on lifestyle changes to manage heart failure.

Your interventions included administering medications, monitoring vital signs, restricting fluids, and providing education on diet and exercise.

During the evaluation phase, you collect data on the client’s vital signs, urine output, and symptoms. You also conduct a physical assessment to check for signs of fluid retentio
